Open these forms with Adobe Reader which can be downloaded FREE using the link at the bottom of this and all pages at the SAS website.
Key your information into the form fields as required and then print out the forms and send them by mail or drop them off at the school office.
If you wish to keep a copy for yourself, print two because you will not be able to save an electronic copy of the form with your information filled into the fields.
If you are a Saint Andrew Catholic Church parishoner and wish to request the discounted tuition rate, please submit the Tuition Rate Request Form.
If you are a Saint Andrew Catholic Church parishoner and do not meet all the requirements for the discounted tuition rate but wish to apply for the discounted tuition rate, please also submit the Donation Exception Form.
If you are active in another Catholic parish and wish to request the discounted tution rate, please also submit the Parish Certification Form filled in and signed by your pastor.
The Health Report Form should be submitted for ALL new students in whatever grade and for ALL students who wish to participate in the extracurricular sports program in grades 6 through 8.
The Medication Permit should be submitted for any students to whom medications of any kind will be given at school.
The Permission to Publish Form must be submitted for ALL new students to Saint Andrew Catholic School.
